Codes that cannot be a primary diagnosis

A claim whose principal diagnosis CMS will not accept comes back as a Return to Provider. Not denied — returned, so it can be corrected and resubmitted. The cost is the time the period sits unpaid while somebody finds a better code, which in an agency running a thin cash position is felt within a cycle.

There are two separate ways a code fails here, and they are worth keeping apart. The 1603 codes on this page carry an explicit CMS flag marking them unacceptable in the primary position. Separately, 30,902 of the 74,719 codes CMS publishes are assigned to no clinical group at all — roughly 41% — and a 30-day period cannot be grouped from one of those either.

Most of the codes below fall into recognisable families: external-cause codes that record how an injury happened, manifestation codes that require their underlying condition to be coded first, and codes too unspecified to describe a home health episode.
