Frequently asked questions
- Can a PTA do a recert visit in home health?
- No. A PTA can never perform the recertification visit or complete OASIS — the comprehensive assessment is restricted to RN, PT, OT, or SLP under 42 CFR §484.55(b)(3), and a PTA does not meet that requirement. If a PTA is delegated routine visits on that case, the supervising PT — who must supervise the PTA under 42 CFR §484.75(c)(2) — still performs the recertification personally.
- Who can do the OASIS assessment?
- RN can complete the OASIS comprehensive assessment at every timepoint. PT, OT, and SLP may complete it only on a therapy-only case with no nursing ordered — OT specifically only when paired with PT or SLP, since OT alone does not establish Medicare home health eligibility. LPN/LVN, PTA, COTA, MSW, and HHA can never complete OASIS.
- Can a PTA or COTA supervise a home health aide?
- No. PTA and COTA are the disciplines being supervised, not supervisors — they never perform the aide supervisory visit. That visit is performed by "a registered nurse or other appropriate skilled professional" under 42 CFR §484.80(h)(1)(i)(A); CMS interpretive guidance (State Operations Manual, Appendix B) limits that skilled-professional substitute to PT/OT/SLP, and only on a therapy-only case with no nursing on the plan of care.
- What visit types can a PTA or COTA perform without direct supervision on-site?
- Routine visits only, under the supervising PT or OT's delegation and direction — federal law requires that supervision under 42 CFR §484.75(c)(2). The supervising therapist's 30-day reassessment (42 CFR §409.44(c)(2)(i)(B)) satisfies PTA/COTA supervision — there is no separate PTA/COTA supervisory visit.
