What this tool answers

Almost every date in a home health episode is anchored to the Start of Care. Enter that one date and this calculator lays out the whole 60-day certification period: when the SOC assessment is due, when the recertification window opens and closes, and which events can interrupt the period along the way.

How to read the result

Day 0 — Start of Care
The anchor for everything else. The comprehensive OASIS assessment must be completed within 5 days after the SOC date.
Interrupting events
A patient returning from a qualifying inpatient stay triggers a Resumption of Care, completed within 2 calendar days of their return. A transfer to an inpatient facility triggers a transfer assessment within 2 calendar days of learning of it. Neither is scheduled in advance — they are shown so you can see where they land if they happen.
Days 56 to 60 — the recertification window
The recertification assessment must be completed inside this five-day window. Missing the back end risks a gap in certification; completing it before day 56 means the assessment does not describe the patient at the point it is meant to.
Transmission deadlines
Each assessment carries its own submission deadline, measured from the completion date in M0090 rather than from the visit.

Common questions

How long is a certification period?

60 days. The recertification assessment that carries the patient into the next period is completed in the last five days of the current one, days 56 through 60.

What if the patient goes into hospital mid-period?

A transfer assessment is completed within 2 calendar days of learning of the transfer. If the patient later returns to your care, a Resumption of Care assessment is completed within 2 calendar days of that return. The certification period itself does not restart.

Does the 5-day SOC window mean business days?

The completion rule is 5 days after the SOC date. The other timepoints in this tool are expressed in calendar days, which is how the guidance manual states them — a weekend does not extend a 2-calendar-day window.

Why does M0090 matter so much?

M0090 records the date the assessment was completed, and every transmission deadline is measured from it rather than from the visit date. An M0090 that does not match reality moves the submission deadline with it.
