TL;DR
- M0090 (Date Assessment Completed) is the date everything else pivots on. Get it wrong and the completion window check, the 30-day transmission clock, and the cert period math all fall apart.
- Completion windows vary by RFA: 5 days for SOC, 2 days for ROC, last 5 days of cert for Recert, 2 days for Transfer, 2 days for Discharge or Death at Home.
- After the assessment is completed, you have 30 calendar days from M0090 to transmit the record to iQIES. Late records hurt HH-VBP scoring and trigger CASPER flags.

Day 14 · RFA 03
Resumption of Care window
If the patient returns from a 24+ hour inpatient stay, complete within 2 calendar days.
Day 30 · RFA 06 or 07
Transfer event window
Transfer assessment within 2 calendar days of learning of the transfer.
Day 56 · RFA 04
Recertification window — day 56
Recert OASIS may be completed during the last 5 days of the cert (days 56–60).
Day 60 · RFA 04 or 09
Recert due — day 60
Recert must be completed by day 60 to start the next cert. Otherwise discharge with RFA 09.
What is M0090?
M0090 (Date Assessment Completed) is the date the assessing clinician completes the comprehensive assessment, including all OASIS data items applicable to the assessment timepoint. The date should reflect the actual date the assessment was finished, not the date data entry was completed in the EHR.
Completion windows per assessment type
The comprehensive assessment must be completed within the following windows: SOC (RFA 01) — within 5 calendar days after the SOC date; ROC (RFA 03) — within 2 calendar days of the patient's return home or knowledge of the return; Recertification (RFA 04) — during the last 5 days of the current 60-day certification period; Transfer (RFA 06/07) — within 2 calendar days of learning of the transfer; Discharge (RFA 09) — within 2 calendar days of the discharge date; Death at Home (RFA 08) — within 2 calendar days of the date of death.
The 30-day transmission rule
Each completed OASIS record must be transmitted to iQIES within 30 calendar days of the M0090 date. Records transmitted after the 30-day deadline are accepted but flagged as late submissions in CASPER reports and may affect HH-VBP performance measurement.
Lock and unlock rules
An OASIS record is considered locked once the agency confirms transmission and iQIES returns an accepted status. After lock, modifications require either a correction record (modifying specific fields while preserving the original) or an inactivation request (removing the original record from the database). The correction window is generally 30 days from the date of acceptance, though specific item-level rules apply.
