OASIS-E2 Item Reference

Section-by-section guidance on every OASIS-E2 item — what it measures, how to score it, common errors. This is Phase 2 of the OASIS Manual.

OASIS-E2 is organised into lettered sections, and the letter tells you what kind of information the item collects rather than how important it is. Section A is administrative — who the patient is, which agency, which payer. Section M holds the clinical and functional items that most directly drive payment. Section GG holds the self-care and mobility items scored on their own six-point scale. Sections C, D and J cover cognition, mood and health conditions including falls.

Knowing the sections is practical, not academic. Skip patterns operate within them, the functional impairment level that feeds PDGM is scored from a specific handful of items, and a validation error at submission is reported by item code — so the fastest way to find what went wrong is usually to know which section the code belongs to. Each section below links to the items it contains, with response options, scoring rules, and the errors that most often send a record back.
