What does 3W2 mean?

Home health frequency shorthand is [visits]W[weeks] — visits per week, then how many weeks that pace runs. 3W2 is 3 visits a week for 2 weeks (6 visits total). Plans of care usually chain several blocks together to taper frequency down as a patient improves, e.g. 3W2, 2W4, 1W3.

Word qualifiers work the same way: QW = once a week, BIW = twice a week, TIW = three times a week, and QD/daily = every day.

What does 3W2 mean?
3W2 is home health frequency shorthand for 3 visits per week for 2 weeks — 6 total visits. The number before the "W" is visits per week; the number after is how many weeks that pace continues. Plans of care often chain several blocks together (a "taper"), like 3W2, 2W4, 1W3, to step frequency down as a patient improves.
How many visits is 3W2 2W4 1W3?
17 visits across 9 weeks: 3W2 is 6 visits (3 × 2), 2W4 is 8 visits (2 × 4), and 1W3 is 3 visits (1 × 3). 6 + 8 + 3 = 17.
Can frequency exceed the certification period?
A frequency order shouldn't run past the 60-day certification period it's written for. If the total weeks add up to more than what fits inside 60 days, the visits past that point aren't covered by the current certification — the physician needs to either recertify for a new episode or the plan of care needs to be rewritten to fit inside the existing one.
What happens when a visit is missed?
A missed visit means the frequency on the plan of care wasn't met, and that needs to be documented — either by making it up within the same week or by documenting the clinical reason it wasn't needed.
