How to score it

Assess ability to move from a seated position to lying down on the bed. Score the level of assistance typically required. Setup (05) means the helper only adjusts pillows/blankets — the patient performs the movement. See Guidance Manual Ch. 4 GG section for full rubric.

The common error

Scoring 'setup' (05) when the helper is actually providing physical stabilization during the movement — that should be 'supervision or touching assistance' (04) at minimum.
