How to score it
Assess the patient's ability to come to standing from sitting in a chair, wheelchair, or on the side of the bed. Code 05, Setup or clean-up assistance, if the only help needed is a helper retrieving an assistive device (e.g., a walker or ankle-foot orthosis). If a mechanical lift is used and the patient still can't reach standing, use the appropriate activity-not-attempted code rather than the 6-point scale; if a sit-to-stand lift is used and two helpers are required to get the patient upright, code 01, Dependent. The common error
Coding 01, Dependent, any time a mechanical lift is involved — CMS distinguishes between a lift that still gets the patient to a standing position with two helpers (code 01) and a lift used because the patient cannot reach standing at all (an activity-not-attempted code, not 01).
