How to score it
Assess the patient's ability to transfer in and out of a car or van on the passenger side; any available vehicle may be used for the assessment. Excludes getting to/from the vehicle, opening/closing the door, and fastening the seat belt. Setup/clean-up of a device used only for walking to and from the car — not for the transfer itself — isn't considered. If the patient stays in a wheelchair and never transfers into the vehicle seat, use the appropriate activity-not-attempted code. See Guidance Manual Ch. 3 GG item-by-item tips for full scoring rubric.
The common error
Treating this item as unscoreable when the patient's own vehicle isn't available — CMS allows any vehicle model to be used for the assessment, so a stand-in car can support a real performance code rather than defaulting to a not-attempted code.
