How to score it
Assess the patient's ability to move from sitting on the side of the bed to lying flat on the bed, or on their preferred/necessary sleeping surface if they don't sleep in a bed. If the clinician determines bed mobility can't be assessed because the head of the bed must stay elevated for the patient's medical condition, use the appropriate activity-not-attempted code (07/09/10/88) instead of the 6-point scale. The common error
Assuming the item can't be assessed simply because the patient doesn't use a standard bed — CMS directs the clinician to assess the sit-to-lying transition on whatever surface is the patient's preferred or necessary sleeping surface, not to default to a not-attempted code.
