How to score it
Assess the patient's ability to maintain perineal hygiene and adjust clothing before and after voiding or having a bowel movement. If the patient manages an ostomy, this includes wiping the stoma opening but NOT managing the ostomy equipment itself. Score usual performance, not best or worst.
The common error
Confusing this item with toilet TRANSFER (getting on/off the toilet or commode) — GG0130C is specifically about hygiene and clothing management, not the physical transfer. Also: including ostomy equipment management (emptying/changing the pouch) in the score, when only wiping the stoma opening is in scope.
