How to score it

Assess the patient's ability to maintain perineal hygiene and adjust clothing before and after voiding or having a bowel movement. If the patient manages an ostomy, this includes wiping the stoma opening but NOT managing the ostomy equipment itself. Score usual performance, not best or worst.
