How to score it

Score ability to transfer onto/off toilet or commode, manage clothing, and perform hygiene. Rate usual performance. If the patient uses a bedside commode independently, the task still rates at the level of assistance needed. See Guidance Manual Ch. 4 GG item-by-item tips for full scoring rubric.

The common error

Not accounting for all sub-components: the item includes transfers, clothing management, and hygiene — not just the physical transfer. Score the lowest level of independence across all components.
