How to score it
Assess the patient's ability to use suitable items to clean teeth, including inserting/removing and soaking/rinsing dentures if applicable. Score using the 6-point scale for usual performance; if oral hygiene isn't observed during the visit, base the code on patient/caregiver report, collaboration with other agency staff, or assessment of similar activities. For a patient who is edentulous, code based on the type and amount of assistance needed to clean the patient's gums. See Guidance Manual Ch. 3 GG item-by-item tips for full scoring rubric.
The common error
Treating an edentulous patient as automatically not scorable — CMS says to code based on the assistance needed to clean the patient's gums, not to skip the item or default to 09/88 just because there are no teeth or dentures to manage.
