Response options

ValueMeaning
06Independent — patient completes the activity by themselves with no assistance
05Setup or clean-up assistance — helper sets up or cleans up; patient completes activity
04Supervision or touching assistance — helper provides verbal cues or steadying assistance
03Partial/moderate assistance — helper does less than half
02Substantial/maximal assistance — helper does more than half
01Dependent — helper does all of the effort
07Patient refused
09Not applicable — patient did not perform this activity prior to the current illness
10Not attempted due to environmental limitations
88Not attempted due to medical condition or safety concerns

How to score it

Score the patient's usual performance during the assessment period for eating (using utensils to bring food to the mouth, chewing, and swallowing). '06' is fully independent; '01' is fully dependent. Use response codes 07/09/10/88 for special circumstances — do not leave blank. See Guidance Manual Ch. 4 GG item-by-item tips.

The common error

Conflating setup assistance (05) with supervision (04). Setup/clean-up (05) means the helper only arranges items or cleans up — the patient does all active eating. Supervision (04) means the helper is present and providing cues or steadying contact during the activity.
