How to score it
Assess the patient's ability to wash, rinse, and dry the face, upper and lower body, perineal area, and feet — excluding the back and hair, and excluding transferring in/out of the tub or shower. May be assessed in a shower, bathtub, at a sink, or in bed (full-body sponge bath). If a helper only retrieves/sets up supplies or applies wound/device water-protection before or after bathing, code 05, Setup or clean-up assistance. The common error
Folding the transfer in/out of the tub or shower into this item's score — GG0130E excludes that transfer entirely, as well as washing the back and hair, so assistance with those tasks should not lower the code.
