How to score it
Assess the patient's ability to dress and undress above the waist, including fasteners. Items to consider include bras, undershirts, button-down and pullover shirts, sweaters, and pajama tops. Supportive devices worn on the upper body — a TLSO, abdominal binder, back brace, stump sock/shrinker, or an arm prosthetic/orthotic — count as clothing for this item. Code based on the type and amount of assistance needed across all garments and devices, even if some are put on at a different time of day than others. The common error
Not counting an orthosis or prosthetic device (e.g., a TLSO, abdominal binder, or arm prosthetic) as a piece of clothing when determining the assistance level — CMS treats these supportive devices the same as any other upper-body garment for this item.
