How to score it
Record the discipline of the clinician who completed the comprehensive assessment. Per the Medicare Conditions of Participation (42 CFR 484.55), the OASIS may only be completed by RN, PT, SLP, or OT — never by an LPN, PTA, OTA, MSW, or aide. The signing clinician's discipline must also be allowed for the specific RFA (e.g., therapy disciplines may complete assessments only when therapy is the qualifying service).
The common error
Entering the discipline of the case manager when a different clinician actually performed the assessment. M0080 is the discipline of the clinician who completed the visit, not the agency's primary clinician of record.
