How to score it
Record the 5-digit (or 9-digit ZIP+4) postal code of the patient's current residence where care is being delivered. Combined with M0050, this drives the wage-index geographic adjustment in PDGM payment calculation.
The common error
Using the patient's mailing-address ZIP when care is delivered at a different address. M0060 must be the ZIP of the location where the clinician visits.
