How to score it
Record the patient's state-issued Medicaid identification number if applicable. Format varies by state. If the patient is not enrolled in Medicaid, use code NA. For dually eligible patients (Medicare + Medicaid), record both M0063 and M0065.
The common error
Using NA for a dually eligible patient because Medicare is primary. Both M0063 and M0065 should be filled when the patient has both coverages.
