How to score it
Record the patient's date of birth in MM/DD/YYYY format as documented in the medical record. iQIES validates this against the date on the Medicare/Medicaid eligibility file; mismatches cause submission warnings.

Entering today's date or the assessment date by mistake. M0066 is the patient's birth date — verify against the medical record before submission.
