M1311 — Current Number of Unhealed Pressure Ulcers/Injuries at Each Stage

How to score it
Report the current count of unhealed pressure injuries at each NPIAP stage (Stage 1 through 4, unstageable, and deep tissue injury). Count only pressure injuries — not other wound types. Do not 'back-stage' a healing pressure injury. See Guidance Manual Ch. 3 item-by-item tips for the full staging rubric.
The common error
Back-staging a healing Stage 3 or Stage 4 wound to Stage 2 or Stage 1. Once a pressure injury reaches Stage 3 or 4, it remains at that classification even as it heals — report it as a healing Stage 3/4, not a lower stage.
