How to score it
Physical assessment and observation are the preferred method; patient/caregiver report and medical records/physician orders may supplement. Code 1 only if both an observable and an unobservable stasis ulcer are present; code 2 if observable ulcers only; code 3 if unobservable ulcers only (present but hidden by a non-removable dressing/device such as a cast or Unna boot). Stasis ulcers exclude arterial lesions/ulcers. Coding 0 or 3 skips ahead to M1340. Dash is not a valid response for this item.
The common error
Reporting an arterial ulcer, or a wound of mixed/unclear etiology, as a stasis ulcer here — CMS's item intent is specifically venous-insufficiency ulcers, and arterial lesions/ulcers are explicitly excluded from this item.
