How to score it

Enter the number of Stage 1 pressure injuries present at the first skin assessment completed at SOC/ROC, coding 4 for four or more. Determine presence by physical assessment and observation — medical records, patient/caregiver report, and physician/allowed practitioner input may be used only for historical purposes (for example confirming a previously healed ulcer's prior stage), not to establish a current Stage 1 injury. Collected at SOC and ROC only. Dash is not a valid response for this item.

The common error

Treating a Stage 1 pressure injury as already healed because the skin is intact — CMS's guidance is explicit that although Stage 1 injuries present as closed, intact skin, they are not considered healed and must still be counted here.
