How to score it
Determine presence via physical assessment and observation. Code 0, No, only if the only pressure ulcer/injury present is a Stage 1 injury, a fully healed ulcer of any prior stage, or none at all. Code 1, Yes, if the patient has any unhealed Stage 2, 3, or 4 pressure ulcer, or any unstageable pressure ulcer/injury (covered by a non-removable dressing/device, obscured by necrotic tissue with no Stage 4 structures visible, or presenting as a deep tissue injury). Coding 0 skips ahead to M1322 at SOC/ROC (and to M1324 at Discharge). Dash is not a valid response for this item.
The common error
Counting a wound whose primary cause is not pressure (for example a stasis or arterial ulcer) as a pressure ulcer/injury here — CMS's coding tip is explicit that if pressure is not the primary cause of the lesion, it should not be reported under this item.
