How to score it
Record the branch identification number assigned by the State agency for the office providing care. If the agency has no branches, this field is left blank or filled per state guidance. Combined with M0014 (Branch State), this identifies the office of record.
The common error
Leaving the field blank when the agency does have a state-assigned branch ID. State surveyor reports rely on M0014 + M0016 to attribute outcomes to the correct office.
