How to score it

Once the patient is seated in a wheelchair or scooter, assess their ability to wheel at least 50 feet and make two 90-degree turns (in the same or different directions, occurring at any point in the distance). A helper may assist with distance or turns as needed; if the patient uses both a manual and a motorized wheelchair/scooter to complete the activity, code based on whichever type requires more assistance. The common error

Scoring based on the easier of two wheelchair types when a patient uses both a manual and a motorized chair during the assessment period — CMS requires coding based on the type of wheelchair/scooter with which the patient needs the most assistance, not the one that produced the best performance.
