Response options

ValueMeaning
06Independent
05Setup or clean-up assistance
04Supervision or touching assistance
03Partial/moderate assistance
02Substantial/maximal assistance
01Dependent
07Patient refused
09Not applicable
10Not attempted due to environmental limitations
88Not attempted due to medical condition or safety concerns

How to score it

Assess the patient's ability to bend/stoop from standing to pick up a small object (e.g., a spoon) from the floor; the activity must be attempted from a standing position, so a patient unable to stand gets an activity-not-attempted code rather than a performance code. Assistive devices, such as a cane for balance or a reacher, may be used. If the only help is a helper retrieving a device like a reacher, code 05, Setup or clean-up assistance; if a helper must pick up the object itself, code 01/02/03 depending on how much of the effort the helper provides. See Guidance Manual Ch. 3 GG item-by-item tips for full scoring rubric.

The common error

Coding a patient who never achieved standing as 01, Dependent, instead of using an activity-not-attempted code — the item requires the attempt to start from standing, so if standing itself didn't happen, the code should reflect that the activity wasn't attempted, not that a helper did all the effort.
