Response options

How to score it
Check every characteristic that applies, drawing on patient/caregiver interview, physical assessment, physician consultation, and the clinical record. If none apply, check only code 10 (None of the above) — no other response is checked alongside it. Dash is not a valid response for this item.
The common error
Counting an inpatient rehab (IRF), psychiatric hospital, or long-term care hospital (LTCH) stay toward “multiple hospitalizations” (response 3). CMS defines that response as acute-care hospitalizations only — 24+ hours in an inpatient acute bed for reasons other than diagnostic testing — and explicitly excludes IRF, psych, and LTCH stays from the count. Response 4 counts hospital ED visits only, not urgent care or walk-in clinic visits.
