How to score it
Check every inpatient facility the patient was discharged from in the 14 days immediately before the SOC/ROC date, where SOC/ROC counts as day 0 and the day before it as day 1 — a discharge on day 0 itself counts. A patient can have more than one facility checked (e.g., hospital then rehab facility). Facility type generally follows the facility's state license, EXCEPT for code 2 (SNF/TCU), which additionally requires the patient actually received Medicare Part A skilled care there — a swing-bed hospital in particular can produce code 1, 2, or 3 depending on which bed/care level the patient actually occupied, not just the facility's overall license.
The common error
Coding a Medicare-certified SNF stay as code 1 (Long-term nursing facility) or vice versa. Code 2 (SNF/TCU) requires ALL THREE of: discharge from a Medicare-certified SNF, receipt of Part A skilled care while there, and that Part A skilled care falling within the 14 days before HH SOC — if any of those three is false, it's code 1 instead.
