How to score it
Check every diagnosis below that is both physician-documented (or documented by an NP, PA, clinical nurse specialist, or other licensed staff where state law allows) AND active at the time of assessment — meaning it currently affects the patient's function, cognition, mood/behavior, medical treatment, monitoring, or risk of death. A diabetic patient who also has PAD or PVD gets BOTH 1 and 2 checked. Dash is a valid, rare “no information” response.
The common error
Inferring a diabetes diagnosis from a documented complication alone — CMS is explicit that finding “neuropathy” or “nephropathy” in the record, with no physician-documented diabetes diagnosis, is not sufficient to check code 2; the underlying diabetes diagnosis itself must be documented.
