How to score it
Enter the most recent inpatient discharge date within the 14 days immediately before the SOC/ROC date (SOC/ROC = day 0, the day before it = day 1; a discharge on day 0 counts). If the patient was discharged from more than one facility in that window, use the most recent of those dates, not the earliest. Dash is not a valid response for this item.
The common error
Entering an earlier facility's discharge date when the patient passed through more than one inpatient stay in the 14-day window — M1005 always takes the single most recent discharge date, even if M1000 has multiple facility types checked.
