How to score it

Enter the date the physician/allowed practitioner order specifically named to start or resume home care, regardless of the discipline ordered. If a revised order delays that date (e.g., extended hospitalization), use the date from the revised order instead — but only if the revised order was received on or before the previously ordered date. A date entered here skips the form to A1255, Transportation.

The common error

Entering the original physician-ordered date after a later revision superseded it. If the order to extend the SOC/ROC date arrives after the previous ordered date has already passed, CMS requires coding M0102 as NA and reporting the original referral date in M0104 instead — not backdating M0102 to the stale order.
