How to score it
Ask the patient: 'In the past 12 months, has lack of reliable transportation kept you from medical appointments, meetings, work or from getting things needed for daily living?' Score the patient's response. This is a standardized social determinants of health (SDOH) screening item adapted from NACHC's PRAPARE tool. Required at SOC, ROC, and Discharge.
The common error
Inferring the answer from documentation instead of asking the patient directly. A1255 is a patient-reported item — clinician judgment cannot substitute for the patient's own response. If the patient cannot answer, use code 8 (unable to respond), not 0 or 1.
