How to score it

Check all race categories that apply — this is a check-all-that-apply item, not a single choice. Ask the patient directly; use proxy input or the medical record only if the patient cannot respond, coding X alongside those responses. Code Z only when it's affirmatively determined none of the listed categories apply. Code Y when the patient declines to respond.

The common error

Treating this like a single-select field and stopping at the first matching category, when a patient who identifies with more than one race should have every applicable box checked.
