How to score it
Conduct a complete drug regimen review — medication reconciliation plus a review of every medication the patient is taking (prescribed and OTC, including supplements, vitamins, and herbal/homeopathic products, by any route) plus TPN and oxygen — as close to the actual SOC/ROC time as possible. Code 1 (Yes) only when, in the assessing clinician's professional judgment, an identified issue reaches the level of clinical significance that warrants physician/allowed practitioner notification by midnight of the next calendar day at the latest.
The common error
Coding 0 (No issues) when part of the review was actually skipped rather than completed and clean — CMS's own coding example treats a review missing even one element (such as the drug-drug interaction check) as incomplete, calling for a dash, not a 0.
