M2003 — Medication Follow-up
Response options, how to score it, and the error that most often sends the record back. Cited to Ch. 3, Item M2003.
Response options
| Value | Meaning |
|---|---|
| 0 | No |
| 1 | Yes |
| - | Dash — no information; CMS expects dash use here to be rare |
How to score it
Applies only to potential/actual clinically significant medication issues identified during the SOC/ROC drug regimen review (M2001). Code 1 (Yes) only if BOTH two-way communication with the physician/allowed practitioner (or designee) AND completion of the prescribed/recommended actions, to the extent possible, happened by midnight of the next calendar day after the issue was identified.
The common error
Coding Yes when communication happened on time but the recommended action wasn't completed (or attempted to the extent possible) until after the midnight-next-day deadline — both communication AND action must clear the deadline for a Yes.
