How to score it
Identify all high-risk medications on the patient's reconciled medication profile (per an authoritative source such as ISMP, the American Geriatrics Society, or The Joint Commission), then confirm the patient/caregiver received instruction on special precautions and on how/when to report problems for every one of them. Code NA only when there are no high-risk drugs in the profile, or the patient/caregiver is already fully knowledgeable about all of them — not simply because education wasn't completed. Dash is not a valid response for this item.
The common error
Defaulting to NA when education just wasn't given, instead of coding 0 (No) — NA is reserved for 'no high-risk drugs exist' or 'already fully knowledgeable,' not as a stand-in for an incomplete task.
