Response options

ValueMeaning
0Able to get to and from toilet independently with or without device
1When reminded, assisted, or supervised by another person, able to get to and from toilet
2Unable to get to and from toilet but is able to use a bedside commode (with or without assistance)
3Unable to get to and from toilet or bedside commode but is able to use a bedpan/urinal independently
4Is totally dependent in toileting

How to score it

Assess the patient's ability to transfer to/from the toilet and perform associated clothing management. Score usual performance, not best or worst. If patient uses a bedside commode or bedpan independently, that determines the ceiling of independence. See Guidance Manual Ch. 3 for full item guidance.

The common error

Scoring only the physical transfer and not considering clothing management and hygiene components that are part of the full item definition.
