| 1 | When reminded, assisted, or supervised by another person, able to get to and from toilet |
| 2 | Unable to get to and from toilet but is able to use a bedside commode (with or without assistance) |
| 3 | Unable to get to and from toilet or bedside commode but is able to use a bedpan/urinal independently |
| 4 | Is totally dependent in toileting |
How to score it
Assess the patient's ability to transfer to/from the toilet and perform associated clothing management. Score usual performance, not best or worst. If patient uses a bedside commode or bedpan independently, that determines the ceiling of independence. See Guidance Manual Ch. 3 for full item guidance.
The common error
Scoring only the physical transfer and not considering clothing management and hygiene components that are part of the full item definition.
