How to score it
Ask the patient: “Over the past 5 days, how much of the time has pain made it hard for you to sleep at night?” Patient interview item — code the patient's own report, not clinician observation. A code of 0 skips the remaining pain-interview items (to M1400 Short of Breath at SOC/ROC; to J1800 Any Falls at DC). Code 8 only when the patient cannot answer.
The common error
Coding from clinician judgment or caregiver report instead of asking the patient the scripted question. The J pain items are patient-interview items; substitute observation only via the structured 'unable to answer' path.
