How to score it
Count falls since the most recent SOC/ROC that produced no evidence of injury on assessment, no patient complaint of pain or injury, and no change in behavior afterward. Code each fall only once, at its single highest level of injury across J1900A–C — a fall with a laceration is coded under J1900B (or C), never split across A and B. Two or more non-injurious falls caps at code 2 rather than an exact count.
The common error
Also counting a fall here that already had an injury coded under J1900B or J1900C. Each fall gets exactly one code across the three sub-items, at its highest injury level — a fall is never double-counted as both “no injury” and “injury.”
