How to score it
This is a two-part PHQ-based mood screening. First ask two stem questions (D0150A1 and D0150B1 — little interest/pleasure and feeling down/hopeless). If either stem is answered '1' or higher, continue to the full PHQ-9 follow-up items. Score each item based on frequency in the past 2 weeks. See Guidance Manual Ch. 3, Section D for administration instructions.
The common error
Conducting the full PHQ-9 even when both stems are '0' (skip logic violation). Also: using '9' (unable to interview) without attempting the interview when A1255 = 1.
