How to score it
Reported per row for the period at or since the most recent SOC/ROC through Transfer/Discharge: b (falls prevention), c (depression intervention), d (pain monitoring/mitigation), e (pressure ulcer prevention), f (moist-wound-healing treatment). Code Yes only when the specific intervention was BOTH included in the physician-ordered Plan of Care AND actually implemented by the time the assessment is completed. Code NA using the row-specific criteria printed in that row's Not Applicable column (for example, row e's NA requires every standardized, validated pressure ulcer risk assessment since SOC/ROC to show no risk) — NA is not a generic 'doesn't apply' catch-all. Dash is not a valid response for this item.
