Response options

How to score it
Observation/demonstration is the preferred method, backed by patient/caregiver interview, environmental assessment, and input from other agency staff with direct patient contact. Grooming covers tasks performed at different frequencies (daily face/hand washing and hair care vs. occasional fingernail care) — weight the response toward the more frequently performed tasks, so a patient independent with daily hygiene but not with fingernail care is scored with MORE ability, not less. If ability varies day to day, enter the response describing ability more than 50% of the time period under consideration.
The common error
Averaging equally across every grooming task instead of weighting toward the ones performed most often — CMS is explicit that a patient able to do frequently performed tasks but not infrequent ones should be considered to have more grooming ability, not less.
