How to score it
This item only addresses ostomies for bowel elimination (for example colostomy, ileostomy) — not urinary ostomies or tracheostomies. If the patient has a bowel ostomy, determine whether it was related to an inpatient stay, or necessitated a change in the medical/treatment regimen, within the last 14 days (the SOC/ROC date counts as day 0). Code 2 if either condition applies; code 1 if the patient has an ostomy but neither condition applies; code 0 if there is no bowel ostomy at all — including one that has since been reversed. Dash is not a valid response for this item.
The common error
Continuing to code 1 or 2 for a bowel ostomy that has since been surgically reversed — CMS's coding tip is explicit that once an ostomy is reversed, the patient does not have an ostomy at the time of assessment, so 0 applies.
